Thefollowingrulesgoverntheuseand/orcompensationforaccruedPTO:AllrequestsforPTOshallbeapprovedthroughproceduresestablishedbytheemployee'ssupervisor.IfaCityrecognizedholidayfallsonaPTOday,theholidayshallnotbecountedagainsttheemployee'sPTObankiftheemployeewouldotherwisebeeligiblefortheholiday.PTOshall,whenused,bechargedinminimumunitsofone-quarter(.25)hour,roundingtothenearestquarterhour.PTOshallbecompensatedattheemployee'sregularbaserateofpay,excludingout-of-classificationpayandotherpremiumandincentivepay.TerminationPayOff:UponterminationofCityemploymenttheemployeeshallbepaidforallaccruedandunusedPTOathisorherfinalbasehourlyrateofpay,uptotheMaximumPayoutshowninthechartabove.TransferfromoneCitydepartmenttoanotherisnotregardedasaterminationofemploymentandtheemployeeisnotentitledtopayfortheaccruedleaveasaresultofthetransfer.Employeesarenoteligibletoreceivepayoffunlesstheyhavecompletedtheirprobationaryperiod.AnnualElection:EmployeeshiredonorbeforeDecember31,2015willhavetheoptiontochangefromthetraditionalvacationandsickleaveaccrualstothePTOaccrualplaneachyearduringopenenrollmenttotakeeffectthefollowingJanuary.Employeesarenoteligibletomovebacktothepriortraditionalvacationandsickleaveaccrualsystemoncetheelectionhasbeenmade.YouwillenterthePTOaccrualscaleatyourcurrentlengthofservicelevel.SickLeaveReserve:EmployeeshiredonorbeforeDecember31,2015andwhotransfertothePTOplanwillhavetheirsickleavebalanceplacedinasickleavereserveaccount.Thisaccountwillnolongeraccrueleave.Employeesmayaccesstheirsickleavereservefortheirownillnessorinjury,orthatoftheemployee'sspouse,domesticpartnerorminorchild,oranypersonlivingintheimmediatehouseholdwhorequirestreatmentorsupervision.Thereservemayalsobeusedforparents,includingstep-parents,parents-in-law,andgrandparents.SpecialcircumstancesmaybegrantedbytheDepartmentHeadwithconsultfromtheAdministrativeServicesDirectororCityAdministrator.Thisreservemaybeaccessedafterthefirsttwenty-four(24)consecutiveworkhoursforeachabsence.PTOshallbeutilizedforthefirsttwenty-four(24)hourworkperiod,andiftheabsencecontinuesbeyond24consecutiveworkhourstheemployeemayaccesstheirsickleavereserve.EmployeeswhoseleaveofabsenceisapprovedasFamilyMedicalLeaveorformedicalcareundertheDomesticViolencePolicymayaccesstheirsickleavereservebankimmediately.Inordertoaccuratelybeconsideredandrecordedasprotectedleave,employeesmustidentifythereasonforandanticipateddurationoftheirabsencewhencallinginundertheirdepartment'scall-inpolicyor scheduleinadvance.EmployeesaccessingtheirreserveunderthiscategorymustalsocontacttheAdministrativeServicesDepartmenttoensurepropertrackingandensureallnecessaryformsarereceivedtoqualifyforuseofsickleavereserve.SickleavereservebalancesareeligibleforpayoffuponretirementmeetingLEOFForPERSretirementrequirementsatarateof25%ofaccruedsickleavetobepaidattheemployee'sfinalbaserateofpay.October2015
