AN ORDINANCE of the City of Camas , Washington,
authorizing the assignment of the franchise granted under
Ordinance No . 2129 to Electric Lightwave , LLC , dba Integra
Telecom of Washington, Inc . , a Washington corporation, and
d franchise for a period of five years ,
extending the term of sal
expiring January 31 , 2014 .
WHEREAS , by Ordinance No . 2129 , the City of Camas , Washington granted unto
Electric Lightwave , Inc . , a Delaware corporation, a five year franchise for the purposes of
constructing , maintaining , operating , replacing and repairing a telecommunications system , and
WHEREAS , Electric Lightwave Inc . has been renamed as " Electric Lightwave LLC , dba
Integra Telecom of Washington, Inc " , and
WHEREAS , both City and Electric Lightwave LLC dba Integra Telecom of Washington,
Inc . , desire to extend the terms of the franchise for a period of five years , expiring January 31 ,
2014 ;
NOW , WHEREFORE , BE IT RESOLVED BY THE CITY OF CAMAS AS FOLLOWS .
Section I
Ordinance No . 2129 shall hereby be amended to authorize the assignment of the
franchisee interest therein from Electric Lightwave Inc . , a Delaware corporation, unto Electric
Lightwave LLC , dba Integra Telecom of Washington , Inc . , a Washington corporation .
Section II
The terms of Ordinance 2129 granting said franchise is hereby extended to January 31 ,
2014 .
Section III
Section 25 of Ordinance No . 2129 shall be amended to provide that notices to Electric
Lightwave LLC , dba Integra Telecom of Washington, Inc . , shall be forwarded to Integra Telecom
Holdings , Inc . , 1201 NE Lloyd Blvd . , Suite 500 , Portland, OR 97232 .
In all other respects , Ordinance No . 2129 shall remain in full force and effect .
This ordinance shall tale force and be in effect five ( 5 ) days from and after its publication
according to law .
